HOW TO MAKE AN APPOINTMENT:
1. Call the office. The best days to call are Mondays, Tuesdays, or Wednesdays; on other days (and times) you may reach our voicemail. If you leave us a message, please leave us your contact and insurance information. We will look up your insurance details and have the information available at the time of the initial appointment.
2. Make an appointment.
3. Download the New Patient Forms and the Clinical Assessment Questionnaire, read and fill out the forms.
4. Please mail the completed forms to the PO Box listed above or scan the completed forms and attach them to a secure email.
Both the iPhone and Android phones have a scan capability. If you do a search for "Scan and email documents from my phone", there are instructions how to do this.
